We got better at writing short monthly decision notes — what changed, pause/keep/kill, named owner, next test. Then a sharper question showed up: how do you know the note actually forced a decision, versus just looking sharper?
Four yes/no checks we use on last month’s note:
If you score 0–1, it was mostly a receipt. Charts don’t fix that.
If you run retainers: what do you score your notes on — and what still fails even when the note looks clean?

Solid framework. One failure mode I'd add to the list: a note can score 4/4 on your checks and still hide a cost problem — the decision was made, owner named, test run and everything, but if that decision involved extra scope or work outside what was originally agreed, and nobody wrote down what it actually cost, it just quietly becomes unpaid work. Might be worth a 5th check: was the $ impact of the decision (if any) captured anywhere trackable, not just the decision itself?
Have you seen whether higher scores on the four checks actually correlate with better client outcomes, or do they mainly tell you the note was executed properly?
