For my bookkeeping business - the worst part is the bad clients. I love the fact that we are able to solve issues for our clients and save them money & time. But sometimes you get a bad apple or two. Unreasonable requests, unreasonable demands, bad manners, and just toxicity. By far, that's the worst part of my job.

The self doubt! I haven't had any sales today, which makes me think I'll never get any sales. When I'm building my app, I worry I'm working on something people will never use. When I'm marketing my app, I worry I'm telling people to use something that sucks.
It's all in my head, of course, but I wish I could be a bit more zen about it.
The lack of light at the end of the tunnel.
As an indie hacker, I find it very hard to mark something as completed. I always see opportunities to improve a product, which makes it feel like I barely finish something... Nevertheless, I never regret it!
Not the worst but arguably the most challenging: managing people
