I highlight the 100% satisfaction guarantee and ask a few questions incl Net Promoter score and also the opportunity to provide a testimonial. Only a small fraction of customers bother to return the form, but those that do invariably are extremely happy and many of those provide a written testimonial. (I flag up front that we would use it for marketing and also tell them we wouldn’t use it without knowing who submitted it.) The survey can be anonymous in case they are otherwise reluctant to supply negative feedback. On the same form, I also encourage them to supply Facebook &/or Google feedback.

I always find it tricky to get testimonials. I usually send couple of emails to users to request testimonials but rarely get any response.

Great! Sounds like you have an elaborate system in place to get those testimonials. What's the % of people who respond back?

Testimonials is something I should take more seriously for Zlappo, but they're pretty easy to get from users simply just by asking.
I prefer to let them fill up an anonymous review/survey form and leave a identifying testimonial if they want.

I guess I could A/B test it, but I haven't.
Logically it can't hurt, and that's good enough for me.
I use them at "friction points" of my funnel, such as the signup page or checkout page.
