Researching "Upgrade plan" UIs

Hi,

I'm working on monetizing MapHub, and I'm trying to figure out a nice UI for asking the users to upgrade.

I wanted to ask it here, as I guess every single SaaS had to implement the same dialog:

  1. Do you know any article or a collection of good and bad examples which I should read?
  2. Can you share a screenshot of your version?
  3. What should I name the title and buttons? "Please upgrade your plan" or "Upgrade required"? "Check out plans" or "Go to pricing"?

Just for reference, I've attached a screenshot of a first prototype. I'd like to move the call-to-action to the bottom and maybe OK/Close should go up in the top right corner as X, as I feel the biggest button to see should be the CTA one.
