We opted for changing the direction we were taking. We had initially thought that a social media type of approach would work for us. But we realised that that would be an issue down the line with people posing as imposter doctors and giving advice to patients that would either be incorrect and/or harmful. So we opted for changing it a bit by adding a mechanism to Validate and Authenticate doctors and their credentials on signup and the other user type would just be left as patient. Then we took away the ability for patients to comment on other patients' questions, while keeping only the readonly and liking (up voting) abilities.
This changed the whole course of the application and we had to think differently.
