What form company is this site using to connect to Stripe Checkout?

Form can be submitted, but if you click an option near the bottom, it will force a Stripe Checkout pop-up to accept payment before allowing the form to be submitted.
Is the form part of Stripe's feature set, in which case it could potentially replace Typeform, or is this a non-Stripe form? If non-Stripe, how complicated is it behind-the-scenes to wire up the identified form to Stripe Checkout?

You embed checkout.js on your page, and when the button is clicked the form opens. If the user puts in some valid card details, Stripe returns a token representing the card, which you can use in a form input that gets sent to your back end.
On your backend, you can use that token to charge the card.
It actually isn't too complicated if you are using stripe's package and handle the payment part in your own backend php file, then you can just update your database with your passed form ID and the payment info etc, hope that helps!
So is the form on this site a package offered by Stripe? That, specifically, was what I was trying to identify.. I am still a bit confused by your response, admittedly.
