Yes, I started a game dev tech business that lasted a few years with a few people I met online, and never met in person. We did it all in IRC and emails. One was deaf so no phone calls for us :)
Yes I have! I co-founded https://cgcookie.com with @wesburke. The first time we meant in person was 2+ years into the project and on the same day that we signed our incorporation papers.
