Anonymous Performance Review Builder

I would like to put together an MVP for an anonymous performance review builder for people management. Any suggestions on any low/no-code options available to do the following:

1. Allow users to give anonymous feedback to their peers and generate performance reports for upper level managers based on input (MVP)
2. Collect the data and eventually develop recommendations based on the data.

Im looking to do something similar to what Ray Dalio has done with the Dot collector (soon to be available on zoom) for those of you have heard of it.
