I try to do follow up calls and emails with every churned person to ask them why they left. If you approach it from a "Hi, I'm trying to learn how to be better" attitude as opposed to trying to sell them etc. You can get some decent response rates. The next part is to help them let their guard down and know it's safe to be honest with you so you can get real feedback.
