🤔 Do big companies have post-mortem?

I have been in several companies. All of them were under 50 people.
I have seen projects going pretty badly but never anyone doing any kind of lookback on what went wrong.

On all my projects, I took the time to think about what worked, what didn't, what I could have done differently....

I was wondering if that's something companies do. What's your experience with this?
