Yes it can be done. The trick is making it HIPAA compliant. There are CMS services now like Caspio, Xano, and Backendless that work well. There is also Knack that can be HIPAA compliant and build out the backend and frontend.
Can even use a service like Doxy for telemedicine.

If you want to call EMR system with no-code, then I recommend reading the blog of Cleveroad https://www.cleveroad.com/industries/healthcare/ehr-emr-patient-portal/, if you don’t know the required code, then they can give you a proper consultation.
I looked at building this last year for our Physical Therapy practice, but the no-code vendors are too expensive. Google Appsheet is also an option, but also not the cheapest either...
I decided to build a standard dashboard in Google sheets with integrated intake forms that is getting interestingly-close to replacing our current (physical therapy) EMR.
The Google Workspace environment can be made HIPAA-compliant on it's own with an executed BAA...So this google sheets dashboard is 100% HIPAA compliant.
The challenging step is to also build a billing solution that can perform the necessary data conversions, post bills, and send/receive EDI data to clearinghouse and insurance payers.
I've shown my MVP to a handful of clinics and there is certainly interest...but there are some non-negligible costs to hosting a full billing solution, the EMR sales cycle is long, and it's a much bigger thing to build...

I am a physical therapist too, I was trying to make a low cost EMR for small local physical therapy centers in my region (Middle East), so I don't really need to make it HIPAA compliant and I don't need a billing solution (I think!).
Unfortunately I couldn't find a cheap way to do it.
