Free (and simple) customer feedback tools?

Hi everyone, does anyone know a free tool to capture simple customer feedback, such as a rating + feedback input pop-up which can be triggered at certain points in the user experience? I'm currently looking at Jotform (https://eu.jotform.com/), does anyone know any alternatives?
