Some of questions needs to be answered before asking people to signup.
Unless you specify which "National provider network" you working with and why people should register with you, it looks spam to me.
Also why should I get in line from your service when you are not affiliated by state governments? Given vaccine distribution strategy would be up to states, how vax.me comes into the picture?
