How serious is the returned/rejected/fake order issue for cash on delivery products?

I have a friend who has an online store and he just mentioned to me that there are a lot of fake orders and returned orders. This problem is very serious in developing countries where most of the orders placed are cash on delivery. I have a solution in mind and its very easy to implement. Just wanted to know if its worth the effort...
