My ex-colleagues created a website for matching blood donors with people who need blood (it's in Bosnian).
But besides posting chats of people they've helped on Facebook, they're not doing much promotion.

Maybe think in terms of epidemics. Who are your super spreaders? You might want to explore establishing a real human relationship with people working in hospitals. Connect with doctors and the red cross. When you can land a partnership with either of these parties you got your super spreaders "infecting" multiple people over time simply because this is how the system is designed. And be it you just drop some flyers at the reception or coffee table somewhere. Like guerrilla marketing tactics. Try cool stickers and t-shirts. People love swag, especially when they can connect to the brand and know why they are doing it. Just note that swag needs high quality. You do not want to save money with swag so be careful with that.

It's a personal project so they don't have the biggest budget but I think they can find sponsors to get resources for swag and posters.

Just try to not sell something. That does usually not work. What you want is to establish a connection to your fellow human beings which is genuine and strong. That does never happen by asking for favours. You want to create your own die hard ultras. Because they will fight for life. I am a bit exaggerating here in order to make a point.
