I just launched in Product Hunt, my app is called Carbos and is free, with a donation option for anyone who wants to help. I decided to do it this way, because it's a very simple app and I created it to help myself and the diabetic community.
Take a look, the app is all about carbohydrate counting for diabetics. Carbohydrate counting helps us know how much insulin we should apply when we are going to eat something.

How did you end up designing the donations UI? What I'm asking is did you have a tip jar kind of page where someone can pick from a list of amounts to donate (via in-app purchase)? Or do you have a link to an external donation page?
Best of luck with the monetization strategy. One monetization strategy that I am going to try next is "pay what you can". I've built a small app recently and am going to release it soon and it'll have locked features, but you'll get to pick from a set of three prices. Each unlocks the same amount, but I want to make it possible for people who don't have much money to still purchase, and those with a bit more to splurge. Not sure how well it work yet, of course.

If you are going on donations, you better file as a non-profit if you haven't. It is viable but you'll be surprised how few people will do it. So think about it - what can you provide to the user that they'd pay for? Could you build a structured diet plan which helps meet their needs, saving them time and brainpower?
