I think somewhere near the top of the landing page you could mention that it's a form survey app. I actually had to watch the video and get 1-min into the video to understand what the product did.
I sort of see this as like a "Typeform" but specifically for customer feedback. Is that correct?

Yes, it's a new way to gather feedback from your customers! We want to get away from that survey idea, and are better off emphasising on the relationships we want you to build with your customers. The initial assumption is that by rewarding and engaging with your customers, you will get more valuable feedbacks.
For the process, it's 2 step: the customer first go through the quantitative questions (ratings), which let us compute an average satisfaction ratio - and we can then ask the relevant questions!

Looks good. I'd just have clearer examples high up on the page. You talk about getting feedback for a club or restaurant, but you could show typical forms for these to really spell it out.
