Is there a simple website for creating employee nps surveys and scores

Hey all - I wanted to create a quick survey to measure my team's general happiness and have heard of employee nps scores. I thought I'd be able to find an easy to use saas website providing enps, but the only stuff I found was expensive enterprise grade tools that aren't a great fit for a small startup like my company.

I'm thinking of building this as my first "indie hacker" side hustle. Is there a simple enps website that exists? If there's something I can find that does the job, I'll use it at work. Otherwise, it's time for a new adventure!
