Landing page to Survey. Feedback required.

Hi there! I have been working on the idea of dynamic digital receipts for a while. Before working on app and software I wanted to check if there is a market for it.
So I created an introductory website and attached a google form for a survey in it.

I would highly appreciate if you can also take the survey as I need at least 500 response to get fair data.
I am not a developer so I did all my best while creating the site. Please, advice some changes you feel will add more value to the site/Feedbacks.

I have noticed that animations on-site sometimes take longer to play. If you face the same thing, please let me know. I will stop them.
