I was previously working at GitBook (a SaaS documentation plaftorm) as a software engineer and I identified this opportunity when we were trying to reduce our churn. This is a cool idea, similar to Baremetrics Churn insights. I think your pricing is on-point. It depends a lot of course on how much your customers charge their customers. If they are B2B then 500 active customers should make them enough money to easily justify $49 for your product. If they are B2C however, it might be very different.
One thing about the cancellation flow itself: please make it extremely easy for customers to cancel their subscription. Forcing the customer to complete a form can lead to a lot of frustration and incorrect responses if the customer just clicks anything to get to the end quicker.
I've also noticed that even on the simplest feedback form, some customers assume that their subscription is cancelled as soon as they click "cancel my subscription". If they don't complete the form after and get charged again they could become very irritated.
One solution would be to actually cancel the subscription immediately when the user clicks the button and then tell them "you have cancelled, would you like to give some feedback"? Then you will only get feedback from people who care to give accurate feedback, you don't have to worry about confused users and if during the flow, the customer decides they don't want to cancel after all, you can just resume the subscription.
Really cool tool. I'll play around with it and see if this is something we can use.

As a co-founder of a subscription based business I must say that I think it looks great!

In my opinion it depends on whether you're targeting consumers or businesses. I think it's important to have an attractive price when you're targeting customers, but not as important when you're targeting businesses.
At the moment, I'm targeting mainly small and medium subscription businesses with 1k-10k customers. A lot of other popular SaaS products can easily cost more than 1k monthly in the same range (Intercom, Hubspot, Pardot, Marketo, etc.). I think most of them can afford a $100/month product if they see the value in it.
It's also easier as an indiehacker point of view of charging 50 x $100 than 200 x $25
