Here’s one from a recent workflow I worked on:

Workflow: AI-assisted feature delivery pipeline (spec → dev → QA → deploy)
What keeps reopening: “Ready for QA” — devs mark it done, but QA expects edge cases + business logic validation not captured in the spec
Where execution slows: Feedback loops bounce between dev ↔ QA ↔ product because “done” isn’t a shared definition

The system keeps moving, but alignment resets every cycle.
