Need Help : How do I create a pop up as an Embedded Script that can be added to any Wix website?

Hey,

I'm building a Wix app where I need to collect user phone numbers and their consent to send out SMS's to their number. Wix has the option to do it via an embeded script.

But I'm struggling to get it working. It would be great if some one could help or could share a Github repo / Stackoverflow article.
