Question of the Day - Knowledge Documentation

What do you dislike, despise, hate, wish was better about technical documentation and knowledge base content creation?

It can be anything: format, code examples missing the copy feature in the top right corner, code examples period, difficulty of searching for an answer, the review process to get updated and new content out and published live, writing the content in markdown or restructured text, anything at all.

What annoys you the most about the process of brainstorming, writing, editing, reviewing, and publishing technical documents or knowledge base content for your business or your work?
