Referral Tracking Platform for lead-gen and Demo requests

Hi everyone, I'm looking for a platform to manage and create a referral program for lead gen and demo requests. Any advice on a simple platform that would allow me to track form submissions (basically a confirmation conversion page) and allow partners to log in and see the status of referral URLs?

The problem I'm having is that the platforms are either geared towards e-commerce with a lot of bells and whistles I don't need or it's geared to enterprise customers above our price range.

Is there a simple referral tracking tool in the $40-50 a month range that would help me accomplish this?

Would appreciate any feedback on my little dilemma:)
