Research Survey: user feedback type

I'm conducting a research on how people give feedback about their experience with an IT product or service (may it be interfaced as a website, mobile or desktop app, whatever) to understand:

1. how much connection with an app people feel
2. why, how and in which cases people actually give feedback
3. whether there're limitations of existing tools/practices of giving feedback
4. whether people feel they help improve the product they use
