Seeking feedback — do you track decision-closure, or just task completion?

Most async teams track task completion. Almost none track decision-closure.

Difference: a task can be "done" while the underlying decision that triggered it was never actually confirmed as final. That gap is where re-work and silent scope creep live.

WODE framework isolates two failure points — Ownership (unassigned) and Decision-closure (undefined trigger for "this is settled").

Curious how other solo/small teams handle this — do you have an explicit closure signal, or does it default to "nobody objected, so it's final"?
