Ours changes every single time - we link to Unsplash to pull a random image for every 404 and 500 error page. Users seem to love it! :) I wrote a blog post about this a while back, which includes some code for anyone who wants to emulate this:

mine is the default netlify 404... also so boring.
BUT when we average 20-30 hits per day I'm not going to worry about it.
My 404 pages are generally the default 404 page of the framework i'm using :P
But seriously, make sure you scan for dead links & use permalinks (or redirect on resource/post delete)
My main cause for 404's is that a) I did some experiment and had google index it 2) decided to stop the experiment and waiting for google to remove it from their index.
IIRC you have to return 404 for google to remove the page permanently, however what I have observed in practice it takes a while (weeks).
