start up lawyers

Hi all - how have people handled business formation here? Am doing a quick survey.

Did you work with a lawyer?
If so, how did you find that lawyer? Did that lawyer do some of the work for free in hopes that they pay it forward and you use them down the road?
Did that lawyer give you a package of startup documents to use? If so, were they customized for your business?
Did anyone use another method?

Any information about how people handle formation and their initial package of legal docs would be greatly appreciated. Thanks in advance!
