User feedback widget - What would you prefer?

I am currently building on https://reportt.app and currently I am in a early research stadium where I want to get as much input as possible from possible users. So that is why I am asking this.

Would you prefer to utilize a feedback widget on your website that allows you to customize the fields users can fill in, or opt for a standardized format (such as feedback, email, file upload)?
