Might sound obvious, but... email.
My approach is basically this: I track what users do my site. Once I notice they do (or don't do) some action in my funnel, I send them an automated email saying "I noticed blah blah. Mind sharing why? It might help us improve." I list 4 numbered options for possible reasons, with the 5th option being "something else".
Many responses are single number answers like "3". I purposely don't link out to a form. I purposely don't add any graphics or links. It is all plain text. My subject is "Super Quick Feedback for <app name>". Response rate is ~25% and this approach also scales as easily as scaling email.
I later realized getting email responses also also helps my email domain reputation, so the rest of my emails are less likely to go to spam. A nice bonus.
What I've noticed is that giving answer options influences users. We used to do this with subscription cancellations emails. Most selected option "I didn't use it anymore".
One we started asking users an open question we started gaining real insights on why they actually cancelled.
You're right. But my feeling is that has more to do with how options are worded vs. giving options. "I didn't use it anymore" is self evident and path of least resistance so obviously it would be picked more.
@dhruvg excellent tip. ill give it a try. Love the email idea. Still learning GA, seeing data, and being able to use it effectively.
