What data should I keep to ensure ToS?

I would like to know what info should I keep for the moment they accept the Terms of Service.
So far I'm guessing
boolean: accept
date: date_of_acceptance
string: IP Adress
Anything else I should keep to be safe and in control in the future?
Contact details. Also if you can log the version history of the TOS they are signing up to this is important if you will be updating your TOS.
