What work in your business never actually gets finished?

I’m noticing a recurring pattern in small teams and solo businesses:

Slack threads that fade out without a decision

Tasks marked “in progress” forever

Follow-ups that everyone assumes someone else handled

Automations that fail quietly and only surface weeks later

Decisions made once, then questioned again because no one remembers why

Not asking about tools.
Not asking about productivity hacks.

I’m trying to understand where work actually disappears.

What kind of work stays open longer than it should?

How do you notice something was never finished?

When something goes wrong, how do you figure out why it happened?
