What's your project risks that keep repeating themselves?

Almost in every R&D project that I work on, there are the same risks keep showing up, e.g. someone taking time off and not finishing their work -- blocking someone else, a big async discussion leading to a decision not being made. I have yet to find solutions that work for each of these consistently.
