Which survey tool with tracking?

I have an early product launched. I'd like to send a survey out via email to a few dozen users. Ideally I'd be able to add a list of emails which would get sent individually (not bcc), user would get a link to a simple survey with a few questions. It would be nice to be able to see which users opened/completed the survey. Looking for something with a free tier or self hosted. Which tools do you recommend for email based surveys?
