your headline lacks specificity. When you mention every business needs form templates, who specifically would you be talking to? A co-founder? A business owner? Someone in Dev? A Growth hacker? You use 80 cents of your dollar when you write a headline so it better attract the right crowd from the get-go. It's fine to exclude people. You want quality leads, not the more the merrier.
check your work through Grammarly or Hemingway app. I don't think the free version of Grammarly tells you about grammar or awkward phrases but Hemingway definitely does.
Your intro is pretty vague. Think about at point your target reader would be at to start thinking about "oh, I think I need to put a form on my site". Try to think in their point of view, not your own. They have different needs. What you may think they need a form for, probably might not be the case.
Only when I get to "complaint form template" do I understand that this article is for business owners. As I mentioned, make that clear from the beginning.
Avoid "weak phrases" like "you would be better off" (instead use "should" otherwise [include undesirable outcome here]) or "would like to know" (instead use "want to know").
All in all, I think you're great at flow, but it's lacking some character. Get your readers to envision a crisis! I need to know WHY THIS IS SO IMPORTANT FOR MY BUSINESS, not just to be told different types of forms.
