July 11, 2026 How I’m bootstrapping a mental health startup in a country of 170M people (where therapy is heavily stigmatized)

Hey Indie Hackers! I’m Imrul, the founder of Chum Wellness.

I wanted to share a bit about what it’s like trying to build and bootstrap a health-tech startup in Bangladesh, an emerging market where mental health care is still considered a massive taboo.

The Market Problem: We have a population of 170 million people, but seeking therapy is often viewed as a "weakness." The market is totally unregulated, meaning vulnerable people often end up getting bad advice from "life coaches" instead of real clinical care.

We built a hybrid platform (telehealth + in-person in Dhaka) where every practitioner is a strictly vetted, licensed Clinical Psychologist.

The Growth Challenge (And our SEO Hack): Because of the heavy cultural stigma, we couldn't just run standard Facebook ads saying "Book a Therapy Session." People are too embarrassed to click them.

So, we decided to completely bypass traditional paid ads and focus purely on Inbound SEO. Here is the scrappy growth hack we used:

  1. Data Scraping: We scraped over 17,000 lines of raw social listening data from local Facebook support groups. We mapped exactly how people describe their psychological pain (the exact local slang they use for panic attacks, toxic in-laws, executive burnout).

  2. The AI Assembly Line: We built an internal AI SOP that takes these raw social pain points and turns them into highly empathetic, clinical-grade blog posts in our local language.

  3. The Moat: We are now mass-generating hyper-targeted, localized SEO content that captures people exactly when they are searching Google at 3:00 AM during an anxiety attack.

Why Bootstrapped? We actively chose not to take VC money right now. In a market like this, VCs want explosive vanity metrics, but mental health requires trust, clinical safety, and slow, sustainable unit economics. We are focusing entirely on profitability and building a high-trust brand first.

I’d love to hear from other bootstrappers:

  1. Has anyone else built a startup in a highly stigmatized or heavily regulated industry? How did you handle your initial Go-To-Market?

  2. If anyone has experience scaling localized programmatic SEO, I’d love to trade notes!

Thanks for reading, and happy to answer any questions about the platform or the Bangladesh startup ecosystem!

I tested the website, it's a bug with the following blue circle, if your mouse goes to the menu blue "Book a Session" after 1sec the dot is destroyed.

What stood out to me is that your biggest insight wasn't about SEO—it was about timing.

People may never click an ad saying "book therapy," but they'll privately search for the words that describe what they're feeling when they're finally ready for help. That shifts your go-to-market from persuading people they have a problem to being discoverable the moment they're ready to acknowledge it. That's a fundamentally different growth strategy.

About

Therapy in Bangladesh is heavily stigmatized and unregulated. I built Chum Wellness to give people a secure platform to connect exclusively with vetted, licensed Clinical Psychologists.
