Physical activity in people with diabetes

The aim of the survey that you see is to help understand what kind of problems, fears and obstacles people with diabetes face, in the context of sports. The information collected will help us work on solutions for people, who would like to benefit from support, whilst taking up the challenge, which is to maintain proper glucose level during physical exercise. Answering these questions shouldn’t take more than 8-12 minutes. The survey is completely anonymous and the information you provide will be analysed collectively. The DiabetesLab Team

At a quick glance, I'd make the value props more prominent. "control your blood glucose level" ... "[optimized] therapy adjustments", etc.
