How do you know when one MVP has turned into several products?

I was on a founder call recently about a telemedicine MVP.

At first, it sounded like one product.
Then the version-one list started growing:

  • Patient app
  • Doctor dashboard
  • Admin panel
  • Messaging
  • Video
  • Billing
  • EHR connection

That was the moment it stopped looking like one MVP. It started looking like several products tied together.

So I grabbed a whiteboard and split everything into 4 buckets:

  • Patient experience
  • Provider workflow
  • Admin tools
  • Future integrations

That changed the conversation fast. The first release got much smaller once we stopped treating all of that like one launch.

What stayed in version one was:

  • Patient intake
  • Appointment booking
  • Secure video
  • Follow-up messaging
    -Basic provider access controls

Everything else moved out.

How have other founders here spot this early? What is the clearest sign that an MVP is no longer one product, but several products hiding in one roadmap?

I wrote up the longer breakdown here if helpful:
