Looking for 1 workflow where everyone thought the work was done.

Usually it wasn't.

The task was completed.
The update was shared.
The decision moved forward.

But people left with different expectations of what completion meant.

One person expected approval.
Another expected review.
Someone else expected a follow-up.

The work finishes.
The expectation does not.

    Workflow: Feature deployment to production.

    What was considered complete: The engineering team finished development, QA approved the feature, and it was deployed successfully.

    What happened afterward: Product expected analytics tracking to be configured, support expected customer documentation, and marketing expected a release announcement. The feature was technically complete, but several teams had different assumptions about what "done" included, so the work was reopened.
